2025 Form 1095-C - Now available in Workday

The 2025 Form 1095-C is now available for you to view and/or print in Workday.

As in previous years, Form 1095-C is provided to all benefit-eligible employees in accordance with the Affordable Care Act. This form includes information about the health coverage offered by Fairfield University and indicates whether you enrolled in that coverage during the year.

Due to a recent change in regulations, printed copies of the 1095-C will no longer be mailed. Instead, you may access your 2025 Form 1095-C at any time through Workday by navigating to Menu > Benefits and Pay > Benefits > ACA Forms.
