At-home COVID test reimbursement information

At-home, FDA-approved COVID tests purchased on or after January 15th, can now be 100% reimbursed through Express Scripts. COVID tests are limited to eight tests per member per month. If one test box contains two tests, the limit is four boxes per member per month. A receipt and completed claim form are required to be sent to Express Scripts in order to be reimbursed. The claim form does not need to be signed by a pharmacist or store representative. Tests can be purchased at an in-network Express Scripts pharmacy or other locations (Amazon, etc.).

Free at-home COVID tests are also available through the Federal Government by visiting this link: https://www.covidtests.gov/. Tests through this site are limited to 4 tests per home and do not count towards the limit listed above.
